Please print these forms, fill them out and either mail or fax them to the PCMI office.  Contact information is listed below.

Due Dates For PCMI Housing and Registration Fees:

April 15th - Registration Form due.  Please note our contact information below.

May 1st - $100.00 deposit for housing is due for non-funded participants and those securing private housing.  This deposit will become nonrefundable after May 15th if you choose to select housing not provided by PCMI, and do not inform our offices by May 15th.

May 15th - Housing deposit becomes nonrefundable if you fail to cancel your PCMI housing request with our office.

May 1st - $500.00 registration fee due.

June 1st - Remaining housing fees are due for non-funded participants and those securing private housing.
